This job is office based, it is not remote.


The Patient Care Coordinator (PCC) is responsible for performing outreach to current and potential Consana patients to set up appointments with Consana clinical pharmacists. The PCC will coordinate scheduling between the patient receiving clinical services and the Consana team. During interaction with patients, the PCC may assist in obtaining necessary patient health-related information needed to perform Medication Therapy Management Services (MTM) accurately and timely within the parameters established by Consana guidelines, state and federal regulations, and per customer requirements.

Primary responsibilities Include

Performs and/or oversees the completion of all routine and tasks, including but not limited to:

  • Make outreach to health plan members and other potential patients daily with the intent of successfully scheduling their appointment with a clinical pharmacist
  • Keep track of all outreach and call notes as required
  • Respond to various patient inquiries as necessary, such as more information about the service offering and potential benefits
  • Provide additional support for administrative tasks, such as maintaining file documentation, making reminder calls, and creating supporting documentation as required by procedures and regulations
  • Assist with follow-up tasks after the appointment, such as helping patients enroll in patient assistance programs, scheduling follow-up encounters and sending documentation of the encounter to the patient or patient’s physician
  • Accept ownership and accountability for the successful completion of assigned functions and achievement of goals

Required Qualifications

  • Strong telephone communication skills and excellent customer service skills required
  • Previous call center or cold calling experience preferred
  • Consistent attention to detail
  • Must be able to work a flexible schedule that may include evenings and/or weekends
  • Strong organization skills including the ability to prioritize and manage multiple tasks in a dynamic environment
  • Strong written and oral communication skills
  • Ability to make independent decisions and able to work with minimal supervision
  • Basic computer skills preferred; experience with Microsoft Excel and Word
  • Ability to work and produce results under pressure
  • Ability to effectively and harmoniously interact with facility, management and other employees at all levels; promotes teamwork and professionalism
  • Must be dependable, have reliable transportation, and be capable of maintaining regular attendance
  • Must be able to speak, read, write, and understand English
